New or existing customers who need to move to another property due to a disability and/or health-related needs must ensure their housing application is fully up to date and supported by appropriate professional evidence.

Have you explored adapting your existing home?

Where possible, adapting an existing home may be a suitable and less disruptive solution. 

You may self-refer to your Local Authority Adult Social Care Department for advice and assistance if you are:

  • a homeowner
  • renting from a private landlord
  • a Housing Association tenant

The Local Authority may arrange for an Occupational Therapist (OT) to undertake a needs assessment within your home.

If your home has been assessed as unsuitable for adaptation

Where an Occupational Therapist has assessed your current home and confirmed it cannot be suitably adapted:

  • ask the OT for a Housing Report
  • the Housing Report must be submitted with your housing application
  • the Housing Report should clearly explain your housing needs and any specific property requirements
  • the OT may also assist in identifying the type of property required

If you have been searching for a home for 12 months or more 

  • your Housing Report may no longer be accurate
  • your circumstances or needs may have been changed
  • your OT may no longer be able to support you in identifying a suitable property
  • a reassessment may be required to ensure housing requirements remain accurate and reflective of your current circumstances

Rehousing based on the identified needs of a child

  • property suitability must be based on an assessment by the Children’s Occupational Therapy Service
  • we cannot determine suitability alone without clinically informed evidence and support from Children’s Services

We will work with Children’s Occupational Therapy Services and other professionals to:

  • understand the child’s assessed needs
  • consider whether a property is suitable
  • explore whether reasonable adaptations could make a property suitable

All decisions are subject to structural feasibility and funding considerations.

Complex Housing Requirements

We will work with Occupational Therapists and relevant professionals to assist in identifying a suitable property to the best of our ability.

Where housing needs are identified as complex, Bromford will require active involvement from relevant health professionals to assist with your application.
